It is the policy of Imaginary Landscape to comply with the standards and guidelines set forth by the United States Department of Health & Human Services, Office of Civil Rights, that govern the obligations and activities of HIPAA Business Associates.

General Business Associate Policies of Imaginary Landscape

• We do not use or disclose Protected Health Information other than as permitted or required by a business associate agreement with our client or as Required By Law.
• We use reasonable and appropriate safeguards to prevent use or disclosure of PHI other than as provided for by an agreement with our client.
• We will report to our client any use or disclosure of PHI not provided for by an agreement with our client of which we become aware.
• We will require contractors or agents to whom we provide PHI to agree to the same restrictions and conditions that apply to us pursuant to an agreement with our client.
• We will make available to the Secretary of HHS our internal practices, books and records relating to the Use and Disclosure of PHI for purposes of determining our client’s compliance with the Privacy Rule, subject to any applicable legal privileges.
• We will respond in a timely fashion to any written requests from our client to provide access to, or make amendments to, any PHI in a Designated Record Set in our possession.

General Precautionary Procedures of Imaginary Landscape

• We do not access or disclose PHI unless requested by our client or directed by law. In almost all cases any such access of PHI by us is used strictly in support of the software itself, as in a request to investigate a reported software-related problem. At those times, access is restricted to necessary personnel only and test data is used whenever possible.
• We employ encryption methods to safeguard data while in transport and while stored on one of our servers. Transport covers both the collection and the delivery of data.

Specific Safeguarding Actions Employed by Imaginary Landscape

• Data transport encryption is accomplished through the utilization of SSL technology.
• While a dataset containing PHI is in the care of Imaginary Landscape, select fields are stored encrypted to prevent associating sensitive information to a specific individual.
• Access to PHI through the Web application, no matter if for viewing or editing, requires an individual user account and password. Group and role accounts are not acceptable.
• All access to PHI via the Web application is logged.
